Please note that all requests may take up to 7 business days for deliverables. Additionally, you may only submit one request per survey. For additional requests, please complete another ACDIS Local Chapter Leadership Request survey.
For CEU requests, ACDIS policy requires that leaders apply for pre-approval at 30 days before the meeting date so that ACDIS can review, approve, and process the request in time for your meeting. This also allows ACDIS National to clarify any questions regarding the agenda that would impact the number of credits approved and post the event on the website in a timely manner.
