Students in need of technology in order to successfully complete courses at ACC may use this form to request a device to check out.

There are multiple steps involved in checking out technology and as a result the process may take a few days, so please plan ahead!

Request Process
There are 2 steps to requesting access to technology before being provided information to pick up a device.
  1. Completing this Student Technology Request Form
  2. Completing a Device Checkout Agreement - the link for this Agreement is available at the conclusion of this request form.

Students must be registered for a credit course, Adult Education, or Continuing Education course prior to completing this technology request form. Failure to enroll in a course prior to submitting this form will result in being denied access to technology.
