**Each family member is welcome to complete their own questionnaire.** Also, if you have already completed your own questionnaire on paper and returned it to the Rectory, please do not duplicate it here.
On the following pages are a few questions - we ask for your honest opinions and recommendations that you wish to share. Your responses are completely anonymous, Should you wish to share your name or other contact information, you may enter it in the "Other Comments" section at the end of the questionnaire.
The summary results will be reported at the Synod on October 7th.
