Please complete this form by 9am on Monday 5th October 2026.

Project Art Works aims to provide the best possible environment for engagement. We use the following information to support each young persons creative journey. Please complete each section as fully as possible.

There is a suggested donation of £10 per family per session.

We will aim to be in contact by Friday 9th October 2025 to offer a session.
