The American Academy on Communication and Healthcare (AACH) is pleased to offer a funding opportunity to participate in a training program aimed toward improving self-management of chronic condition. The AACH extends this offer in a shared project with the Flinders Program from Flinders University Human Behavior and Health Research Unit in Adelaide, Australia. This project is anticipated to last 18 months and training will take place in Allentown, PA.

For the last 15 years the Flinders Program has developed a truly patient-centered structured communication tool based on motivational interviewing that facilitates patients' engagement with self-managing their chronic condition(s), resulting in a negotiated care plan. This communication tool can be applied to any chronic condition including, but not limited to, diabetes, weight management, depression, stress, CHF, alcoholism, cancer treatment, and chronic pain. Selected candidates will receive an award valued at $4500 ($3500 training registration fee + $1000 travel stipend to attend the training).
