AMGA will review each application and use the following criteria for acceptance into the collaborative.
Pre-requisites: Current member of A4i, active use of Optum One, and participation in the Together 2 Goal® campaign. Use of Optum One’s Registry module is a plus, though not essential.
Additional criteria:
· People and resources to support the project, including analytics staff
· Potential impact: number of patients forecasted to improve care
· Willingness to share best practices with other organizations
· Innovative approaches
· Feasibility, sustainability, scalability
