Terms of use
The A-HSOPS 2.0 was modified from the U.S. Agency for Healthcare Research and Quality Surveys (AHRQ) on Patient Safety Culture™ (SOPS®) Hospital Survey 2.0. AHRQ granted the Australian Commission on Safety and Quality in Health Care permission to use the SOPS Hospital Survey 2.0 to develop the Australian modification subject to appropriate attribution of their work. 

With the exception of any material protected by a trademark, any content provided by third parties and where otherwise noted, the A-HSOPS 2.0 is licensed under a Creative Commons Attribution–NonCommercial–NoDerivatives 4.0 International licence.

A-HSOPS 2.0 is available free of charge for non-commercial purposes for use in Australian Hospitals. Allowable modifications to the survey are articulated in the technical specifications for the Australian HSOPS 2.0.
