Ecology Action and County Health Services offer programs to Santa Cruz County area schools to encourage more students to walk and bike to school and do so safely. We are about to submit a proposal to fund active transportation work at local schools. We’d appreciate you taking 5 minutes to fill out this survey so that we can design programs that increase safe, active and green transportation for students.

* 1. What school(s) are you affiliated with? (Write name of school)

* 2. What is your relationship to the school?

* 3. Currently both bike and walking safety is taught during school hours. If we were to move some of these safety presentations outside of the schools hours, what do you think would be the best way to do so:

* 4. The Bike Smart Program involves a bicycle safety classroom/assembly presentation, followed by a hands-on training event. Would you support creating the same program model for walking safety education?

* 5. Would you support a regular yearly bike safety education program serving all 5th graders for every elementary school in the county? If no, do you have other suggestions?

* 6. How would you rank each of these safety efforts in terms of their ability to increase safe biking and walking among students?
