Managing Anaphylaxis in Schools |
Introduction
As a parent of a child with a potentially life threatening allergy, we are requesting your help by asking you to complete this questionnaire that asks about your child and experiences at school related to anaphylaxis.
Your involvement will take about 10 minutes. While completing the survey, you have the opportunity to go back to previous pages/questions and update or change your responses. However, once the survey is completed and you have exited, you will no longer be able to re-enter the survey to make changes. Only one response is allowed per computer.
