Purpose of the Collaborative Vitality Survey: The Collaborative Vitality Survey was developed by Georgia Family Connection Partnership. The Survey is a systematic way for individuals to express their experience of membership in a collaborative. Survey findings can be used to help acknowledge and celebrate collaborative achievements while identifying areas for improvement and growth.

Taking the Survey: Your participation in the Survey is completely voluntary. There is no sanction or reward for participation other than what you learn (see below). It should take about 10 minutes to complete the Survey. The Survey is anonymous other than the county name and your role, so please be candid. It is entirely up to you to decide how to interpret each item on the Survey. If you are not sure what an item means, make your best guess or check “don’t know.” If you are a member of multiple Family Connection collaboratives in different counties you should consider just the collaborative that invited you to complete the Survey.

What next: All completed Surveys will be compiled and analyzed by GaFCP staff. Findings about your collaborative will be returned early in 2017. Your collaborative's findings will not be shared with anyone else.
