It's time to re-enroll in music lessons for Fall 2012.
Fall Session 2012: August 20-January 15 (18 weeks)
To continue your lessons, please follow these steps:
1. Complete this form. You will not be re-enrolled unless you complete this form (or submit a paper copy). This holds your place in your teacher’s studio.
2. To be eligible for continued enrollment, all current charges on your student account must be paid in full by July 30. Initial fall statements will be posted to your online account in mid-August.
3. If you decide to cancel lessons after returning the form, you have until August 19 to call the Cancellation Hotline (314-968-5958). If your cancellation is received after this date, you will be responsible for a $125 cancellation fee.
4. Talk with your teacher about lesson days and times for fall semester.
