CONSENT FOR PARTICIPATION IN A RESEARCH PROJECT

You are invited to participate in a research study designed to look at features and associated characteristics of emotional distress and various other emotional difficulties.
If you agree to participate in this study, you will be asked to complete questionnaires online. If you choose not to answer any question, you may skip the question. The entire survey should take approximately 50 minutes to complete. If at any point you should choose to quit taking the survey, you may simply close the browser and no penalty will come to you.
Risks and Inconveniences: No risk should come to you because of your participation in this study. Some of the survey questions may be sensitive in content. If the content of any of the questionnaire items is overly sensitive and you would like to speak to a professional about borderline personality disorder and/or emotional concerns, please contact one of the following:

Benefits: No direct benefit is expected to come to you as a result of your participation. However, your participation may help science by improving the understanding of emotion dysregulation and coping patterns associated with emotional distress. Further, your participation may help us to be better able to diagnose and to develop treatments for emotional distress and associated problems.
Economic Considerations: In exchange for your participation, if you desire, you will be included in a lottery to win a $20 electronic gift card to Amazon.com. You will have a 1 in 20 chance of winning this prize. If you wish to be included in the lottery, you must provide an email address to which the gift card can be sent. After you have completed the questionnaire, if you chose to be included in the lottery, you will be redirected to another brief survey that will collect your email address. Your contact information (email address) will not be linked to your questionnaire responses in any way. Please note that in order to be included in the lottery, you must provide your email address. Only those participants who provide an email address will be included in the lottery.
Confidentiality: No identifiable information will be obtained in connection with the questionnaire items. Your questionnaire responses will not be traceable back to you. However, if you choose to be included in the raffle drawing, you will be directed to an additional questionnaire that will collect your contact information. Your contact information (email address) will be collected ONLY for the purpose of notifying you that you have won the drawing. Your identifying information will not be connected to your questionnaire responses in any way. Any identifiable information that is obtained in connection with this study will remain confidential and will be disclosed only with your permission or as required by the U.S. or State law.
Your questionnaire responses will be combined with those of other respondents and will be stored in password-protected datasets. It is expected that the results of the questionnaires will be published or discussed in conferences; however, no information that would reveal your identity will be included.
Representatives from the UCLA Institutional Review Board may inspect study records during internal auditing procedures. However, these individuals are required to keep all information confidential.
