From 1st December 2014 it is a contractual requirement that all GP practices undertake the NHS Friends and Family test (FFT). The purpose is to give patients the opportunity to provide feedback on their experience to help improve services.

The first question has been set by NHS England. The results will be sent periodically to NHS England and available on our website from January 2015.

The test should take no more than a couple of minutes and we would be very grateful for your response.

* 1. We would like you to think about your recent experiences of our service at Garstang Medical Centre. How likely are you to recommend our GP practice to friends and family if they needed similar care or treatment?

* 2. Can you tell us why you gave that response?

* 3. Are you happy for us to use your comments publicly, e.g. on the practice website? (anonymously)

We are asking the following questions in order to collect equality and diversity information to ensure we are reaching a representative group of our practice population.

* 4. What is your sex?

* 5. What age are you?

* 6. What is your ethnic group?

* 7. Do you consider yourself to have a disability?

Thank you for taking the time to complete our questionnaire.
