You are invited to take part in a research survey, conducted by librarians at the UT Libraries. The purpose of the research is to measure what you learned at the Big Orange STEM Symposium (BOSS) in order to assess whether the program is memorable and/or useful. BOSS hopes to introduce students to careers in the fields of science, technology, engineering, and mathematics through hands-on presentations.
INFORMATION ABOUT PARTICIPANTS’ INVOLVEMENT IN THE STUDY
You will be asked to complete an online survey with 5 questions before the program and a printed survey with 5 questions after the program. Each should take about 5 minutes to complete. You must be 18 years of age or older to participate or have your parents’ consent.
There is minimal risk for harm or injury. Some discomfort may be felt after spending time sitting in front of a computer and/or typing or writing.
Your participation in this survey will help the UT librarians improve STEM programs provided to students such as yourself. The survey will also help UT librarians/investigators share with colleagues at other universities (in articles or presentations) some strategies that may contribute to effective STEM programs. Benefits of attending the STEM Symposium include:
* The opportunity to learn about research and classes in STEM fields.
* The opportunity to network with professors and scientists in STEM from the community.
* The opportunity to learn about research and academics on UT’s campus and library.
The information gathered in this study is anonymous and will be kept confidential. Data will be stored securely and viewed only by persons conducting the study. No reference will be made in written or oral reports that could link participants to the study. Your teachers and/or counselors will not have access to your responses.
All who complete the surveys will be entered into a drawing for a gift card or prize. All who register for the program will be eligible to receive bags and prizes regardless of whether they complete the surveys.

You do not have to participate in this study, nor is it required to attend the symposium. If you decide to participate, you may withdraw from the study at any time without penalty and without loss of benefits. If you withdraw from the study before data collection is completed, your responses will be destroyed. Must be 18 or over to participate or have parent’s permission.
I have read the above information. I understand that I will be asked to complete a survey at the beginning and end of the program. I understand that completed surveys will be strictly confidential and used for scholarly research purposes by the researchers only. I have received a copy of this form. I am 18 years of age or older or have a parent’s permission. I agree to participate in this study. I understand that I do not have to participate in this study to attend the STEM Symposium.
