Last August, the City of Des Moines restriped Hubbell Avenue between Easton Boulevard and E 33rd Street to a three-lane road (one lane in each direction and a left-turn lane) as part of a pilot project. Before it was restriped, a public survey was conducted to get citizen input.

Now that the restriping project has been in place for about eight months, we would like to get your input on this "follow-up" survey, specifically the following questions. The questionnaire should take you only 5-10 minutes to complete.
