Thank you for taking the time to submit a nomination for the 2015 Council of Unions and Student Programs Awards. Your nomination form must be complete, address all award criteria, and the nominee must meet all criteria of the award. Please be specific about the accomplishments of your nominee. We encourage you to give as much detail as possible since the committee will (most likely) only know the nominee through the provided materials. Please, only one name (or program) per nomination form. You may submit multiple names and programs for each award category.
