1. Invitation to Participate and Description of the Project

This is a voluntary survey of your experiences with emergency contraception. Before agreeing to be part of this study, please read the following information carefully.

Risks: There is a possibility that some of the questions in the survey may make you feel uncomfortable, as they ask you about your personal feelings. I do not expect this to happen, but if you do feel uncomfortable, you can choose to stop the survey at any time before you click the submit button. I have provided information on this website on how to contact counselors in your area should you need to do so.

Benefits: This study is not designed to benefit you directly. I hope that what is learned from the study may provide a better understanding of women and men's diverse experiences with emergency contraception.

Confidentiality: Any and all personal information obtained from you during the study will be confidential. Your privacy will be protected at all times. Your IP address will not be stored with the survey data. The information collected from this survey however, may be used as part of publications and papers related to the history of emergency contraception.
