We are conducting a research study about medical bad news delivery and the communications between patient and care provider. You have been contacted because you have had a HIV positive diagnosis. We would like to offer our apologies if being contacting about your status brings up uncomfortable memories of your experience.
If you feel able to share a few details about your diagnosis, we invite you to answer the following survey questions about your experiences and opinions about when you were diagnosed. We are hoping to learn more about this kind of communication and offer insights to care-providers about the effects of these interactions. We will also ask questions to gather demographic information. Data collected from this confidential survey will be used for research purposes.
You must be 18 years of age or older to participate. If you participate, there is a risk that you might feel some emotional discomfort in answering these questions. We have listed a few support resources below and at the conclusion of this survey, you will also have access to the same resources seen here. You may stop participating at any time and you may refuse to answer any question.
The survey should take approximately 25-30 minutes to complete. If you choose to give permission to be contacted for further research and provide us with your email contact information at the conclusion of this survey, we may contact you for participation in future studies. If you provide us with your email address, your email address will be stored separately from your questionnaire in a confidential and secure electronic file. Your email address will not be stored or associated with your survey responses.
Any questions or concerns should be directed to the principal investigators: David Nalos at firstname.lastname@example.org or Suzanne Pullen at email@example.com or (415) 338-3158. You may contact the research advisor, Christina Sabee, Phd. at firstname.lastname@example.org. You may also reach any of us by mail at Communication Studies, San Francisco State University, 1600 Holloway, San Francisco, CA 94132. Questions about your rights as a study participant, or comments or complaints about the study, may also be addressed to the Office for the Protection of Human Subjects at 415: 338-1093 or email@example.com.
Thank you for your time and consideration,
David Nalos and Suzanne Pullen
