State surveyors provide a valuable service to hospice patients, their families and state regulators, as well to hospice providers by ensuring the delivery of quality hospice care.
In our continuing efforts to improve hospice and end-of-life care and services throughout this part of the country, LMHPCO wants your input in assessing state surveyors' performance and effectiveness each time they interact with your agency.
LMHPCO is requesting its members complete the short survey below, immediately following the completion of your annual and/or follow-up surveys. Your feedback regarding your experience of the survey process, as well as the surveyor is vital to LMHPCO's documentation and understanding of your survey experience.
All responses are strictly confidential and will only be used in the aggregate form each year to provide state regulators with our collective input.
