Dear Parents and Guardians,

We are conducting an evaluation of our school. Thank you for taking the time to reflect on the following questions and answer them honestly. Your responses will help us to improve the experience in all areas.

This survey asks for your "degrees of agreement" with Bishop Mora Salesian High School statements. We would like to know how strongly you agree or disagree with a statement that we offer you.

Also, we want to assure you that your responses to this survey are completely anonymous. Names and IP addresses will not be collected. Your answers will not affect your son's grades or financial aid award.

If you have any questions or concerns, please direct them to Mr. Burke at rburke@mustangsla.org or 323-261-7124, Ext. 320.

Thank you for your cooperation!

Sincerely,

Robert L. Burke, M.A.
WCEA Accreditation Coordinator & Director of Student Activities
