Introduction

This study is to determine the rate, range and severity of side effects and adverse reactions associated with fluoroquinolone antibiotics. It will be the first of many studies we will be conducting on this subject.

Anyone who has been prescribed a fluoroquinolone antibiotic, whether an adverse reaction developed or not, may take the survey. Parents of children, spouses and other family members of individuals who have been injured by fluoroquinolone antibiotics may also complete the survey if the antibiotic recipient is either too young or too injured to complete it themselves.

The survey is anonymous and takes approximately 20-30 minutes to complete. The survey is divided into sections that roughly approximate physiological systems that may be affected by these medications. There is necessary overlap between symptoms clusters.

At the end of each symptom cluster, there is an opportunity to input additional information about that cluster of symptoms. There are multiple clusters of symptoms and so not every symptom will appear immediately. You may move back and forth between sections.

At the end of the survey, if we have missed symptoms, you will be given the opportunity to add information.

Some questions and responses will be specific to females and others to males. In some instances, the software allows us to segregate those questions based on sex but, in other cases, it does not.

Be sure to complete the entire survey, so as not to skew the results.

Results from this study will be used to inform physicians and patients about the fluoroquinolone reactions and will be published online.

Please share this survey with your friends, family, colleagues and anyone you know who has been given a fluoroquinolone antibiotic. We are crowdsourcing this research to find the connections. That requires sharing via social media. We appreciate your support.

Thank you in advance for taking the survey.
