* 1. Consent to Participate

The purpose of this research is to identify supplements you use and feelings about self perception. This is a research project conducted by Dr. Julie Schneider, an assistant professor in the Nutrition and Food Sciences department at California State University, Chico. You are invited to participate in this research because you are a college student older than 18 years of age.

Your participation in this research study is voluntary. You may choose not to participate. If you decide to participate in this research survey you may withdraw at any time. If you decide not to participate in this study or if you withdraw from participating at any time, you will not be penalized.

The procedure involves completing an online survey that may take approximately 20 minutes. Your response will be confidential, and we do not collect identifying information such as your name, email address or IP address. The survey questions will be about your use of dietary supplements and psychosocial factors of self perception.

We will do our best to keep your information confidential. All data is stored in a password protected electronic format. To help protect your confidentiality, the surveys will contain information that will only be used for scholarly purposes and may be shared with California State University, Chico faculty and students in the Nutrition and Food Sciences Department.

If you have any questions about the research study, please contact me via email and I will respond in a timely matter. This research has been reviewed according to California State University, Chico IRB procedures for research involving human subjects.
