IACUC Anonymous Survey to Report Animal Concerns

The IACUC recognizes that it is a privilege to use animals in research and welcomes any information which would aid our efforts to assure the humane care of research animals used a MMCRI.

The IACUC's authority falls under the oversight of the NIH. This committee is responsible for assuring all regulations related to animals adhere to the strict guidelines of the NIH.

If, for any reason, you have concerns regarding animal welfare practices at MMCRI, you may report your concerns in one of several ways:

Speak with the Animal Facility Manager
Speak with your Lab Manager or Principal Investigator
Speak with the Consulting Veterinarian
Call the IACUC Research Coordinator (207-396-8195)

Submit your concerns CONFIDENTIALLY by using this form.

If you have an urgent concern and need a quick response, please contact one of the persons listed above. This form is not intended for issues requiring immediate attention.

Regardless of the way you decide to report your concerns, your name will remain confidential. If you prefer total anonymity, then this form is the means you should use.

Your IACUC concerns will be forwarded to the IACUC Chair and the Attending Veterinarian for evaluation of the situation. The IACUC committee will take appropriate action in a timely manner.

All animal welfare concerns will remain confidential, and without reprisal.

A final report of the findings will be sent to the Associate Vice President of Research and the individual who reported the concern, unless the individual does not provide their name.

THE PERSON MAKING THE REPORT IS NOT REQUIRED TO PROVIDE HIS OR HER NAME, UNLESS THEY WISH TO.
