You are invited to participate in a brief survey to help the substance use disorder treatment community and your provider better understand the impacts of the COVID-19 pandemic on patients and their treatment. This survey is being conducted in collaboration with the OMNI Institute, an independent, non-profit research and evaluation firm. You have been selected to complete this survey because you are currently enrolled in treatment.
Your responses to this survey are important to understand the impact that COVID-19 has had on substance use disorder treatment. The results of this survey will be used to help guide improvement of and adjustments to treatment services as a result of COVID-19.
The responses you provide will be confidential. No names or identifying information will be connected to your responses. All information provided from the survey will be combined with responses from other survey participants and reported to your treatment provider using averages, by OMNI. Your open and honest responses are very important to us. This survey is completely voluntary and you may decide not to answer some or all of the questions.
