Expression of Interest Form

Help us co-design an Accessible Disaster Recovery Toolkit.

We are looking for people with disability, carers, and community members to help design an Accessible Disaster Recovery Toolkit. This form is to collect some basic information about you, so we can make sure we hear from a wide range of people.

You can be involved in two ways:

  • One-to-one interviews – held between 3 and 7 November 2025, with a choice of in person or online
  • Co-design group – three workshops held in person to work together to develop the Accessible Disaster Recovery Toolkit. To join the codesign group, you need to attend all three workshops, held on the following dates:
    • 20 November 2025
    • 4 December 2025
    • 24 February 2026

Your answers will be kept private. You can skip any question you do not want to answer.

This Expression of Interest Form will close on 18 November 2025. Selected participants will be advised via email in mid November.
