CONSENT TO PARTICIPATE IN RESEARCH

I agree to participate in a research study called:
MEASURING PSYCHOLOGICAL FLEXIBILITY, VALUED LIVING AND EMOTION REGULATION THROUGH THE EMOTION EFFICACY SCALE: A VALIDATION STUDY

This study is a dissertation research project being conducted by Max Shannon, a doctoral student at the Wright Institute, in Berkeley, California. The study is being supervised by dissertation chair Dr. Matthew McKay, who can be contacted at matt.mckay@newharbinger.com, and dissertation second reader Dr. Aprilia West, who can be contacted at awest@wi.edu.

The study will explore measurements in emotion regulation. I understand that as part of my participation in this research, I will be completing research questionnaires on an online survey. At the conclusion of the study, I may request a summary description by contacting the researcher, Max Shannon, at 510-629-1504.

I understand my participation is entirely voluntary that any information I provide will be protected to the fullest extent of the law. I am encouraged to read the SurveyMonkey Privacy Policy, and understand that any personal data collected from me may be transferred to various countries, including the United States and other locations SurveyMonkey has offices. 

It is hoped and expected that the data collected may help improve future treatments for individuals who struggle with emotion regulation. I understand that my participation will involve minimal risk and may result in no benefit to me, beyond what might be gained through the experience of participating in a research study and contributing to a better understanding of the topic. My refusal to participate will involve no penalty or loss of benefits.

I understand that any problems or discomfort I may experience, as part of my participation, should be immediately reported to Max Shannon at 510-629-1504 or emailed to him at mshannon@wi.edu.
