· You have no criminal convictions or outstanding criminal charges.
· You are not subject to a finding of impaired fitness to practise by any regulatory body.
· You are not disqualified from a profession or as a company director.
· You are not an undischarged bankrupt, and not the subject of any bankruptcy restriction order.
· You do not have an unspent conviction for a money laundering, bribery or terrorism-related offence, amongst others.
· You have never been found in contempt of court.
· You have never been removed as an officer or employee of a charity for misconduct or mismanagement, and
· You have not been a 'designated person' under anti-terrorist legislation.

I confirm that to the best of my knowledge the information contained in this application is complete and correct. I further confirm that I have considered and understood the criteria for suitability for appointment and I do not fall into any of the categories, listed in the Candidate Information Pack, which would bar me from applying.
