This registration is a two step process.  Upon filling out this form, you will receive an email with specific instructions related to the method of payment you chose - whether that be credit card or by check.  Those instructions will detail the second part of the process of completing your registration.  We are doing this to ensure that credit card registrations are done securely through PayPal.  These same payment instructions, in an abbreviated form, will also be provided at the end of this survey when you click the "Done" button.
 
Registration is not confirmed until payment is received.  Once payment has been recived an email will be sent as confirmation. Unfortunately we will be unable to issue a refund within 30 days of the seminar.
