As rural residents, access to quality, local healthcare is a major concern.  This Community Needs Assessment Survey is a collaboration with Ste. Genevieve County Memorial Hospital, Ste. Genevieve County Health Department, Ste. Genevieve County Community Center, and Ste. Genevieve County University of Missouri Extension Office designed to allow you and other residents to share your opinions about health care services provided in the community.

The information you share will help us determine the types of care and services Ste. Genevieve residents, the people we are here to serve, need and want.

This is a household questionnaire.  Please answer this on behalf of all family members living with you, including yourself.  The process is completely confidential, so please be open and honest, and return your information as soon as possible, but no later than August 1, 2019.
