2020 Goal of the Year Nomination Form

* 1. Criteria:
A Video Clip submitted must:
(1) be submitted as only the play be nominated
(2) not more than 60 seconds long
(3) be a goal that occurred during the season year immediately prior to the seasonal year for which the clip is being submitted. IE any goal scored in 2020 calendar year.
(4) be for a goal in any college club soccer game, 2020 National League, Aztec Cup, or NIRSA Affiliated league like WCSA.

* 4. The Completed Nomination must be post marked by Jan 15th and sent gaming.aztecup@gmail.com. Your completed nomination should include on the following:

(1) Completed MailChimp nomination form
(2) Permission statement (when necessary) - If the person submitting the clip did not film the clip, written permission from the owner of the footage is necessary. The name and contact information of the owner needs to be included as well.
(3) Unedited, original video clip sent via YouTube, email attachment, Dropbox, or other online service addressed to:
