Cloverleaf Schools and the Medina County Health Department are partnering to explore the possibility of having a school and community health center at Cloverleaf Middle and High School. We would like to hear from the community to learn about what services are needed and if residents would like to have a school and community health center in the area. This survey will take approximately 5 minutes to complete. Your answers will be kept confidential.
Health centers provide affordable, high-quality and accessible primary healthcare. Private insurance, Medicaid, Medicare, and self-pay patients are all accepted. Sliding fee scales make services affordable. All ages are welcome. Services are for everyone! We want to gather insights from both community residents and those that have children in Cloverleaf Schools. Any potential health center would be designed to serve both Cloverleaf students and community residents, regardless of affiliation with Cloverleaf Schools.
