Post-Hospital Discharge Care in Wandsworth

This survey was created by the Wandsworth Disabled People's Forum to help capture data about Disabled and Older resident's experiences getting the care they need after they are discharged from hospital. We are particularly capturing people's experiences from the past 2 years.

The information you provide is anonymous and will not be stored with any identifying information about you. We will use anonymised data to help campaign on this issue.
