Informed Consent Form for Survey Research

Before you start answering questions, please read and consent to this survey.
 
Background Information:  The purpose of this study is to assess which factors in underlying team dynamics lead to entrepreneurial companies team effectiveness.  
 
Procedures:  Upon your agreement to participate in this study, you will complete a survey that will take approximately 25-30 minutes.
 
Risks and Benefits Associated with the Study:  This study does not have any known risks. By participating in this study, you may experience the benefit of greater understanding of success factors in working on and in an effective team, as well as reflection of how such teams operate. 
 
Confidentiality: All survey responses will be strictly confidential.  At the beginning of the survey you will be asked for the name of your company.  This is required so the researcher can aggregate the data by company.  Once the data is aggregated, company name will be removed and the data will be anonymized.  Only the researchers will have seen the original surveys. The records of this study will be kept on the researcher’s password protected computer and then stored as mentioned in anonymous form, preventing any breach of confidentiality.  Should the study ever become published material, the name of your company will in no way be linked to the study, nor will it mention your involvement or that of your company. These records will be destroyed after statistical analysis is completed. Your responses will be anonymized.
 
Voluntary Nature of the Study:  Your decision whether or not to participate will not affect your current or future relations with this student researcher, with Benedictine University faculty, or with your company. You are free to withdraw at any time without affecting your relationship with the researcher, with Benedictine University, or with your company.
