CCA Prospective Students

This survey should be completed by parents or guardians who would consider enrolling a child at CCA in the first 5 years of school operation. The school will open with K-6 grades in 2022-2023. 

We are collecting this information to develop accurate estimates of school enrollment. The information will be used by school leadership for planning purposes only, and it will not be shared with other parties. Completing this survey will not enroll your child at CCA, nor will it impact your child's chances to enroll at CCA in the future. 

We request that the survey be completed by one individual per household. We will first ask for your contact information, and then information about your prospective student(s).
