2018/19 House League and Roster Select Coaches Evaluation Survey

Kingston Area Minor Hockey Association HL and RS Coaches Evaluation for the 2018/2109 season

The following survey has been developed with the purpose of evaluating our house league and roster select coaches within Kingston Area Minor Hockey Association.  Your input will help to:

1. Provide feedback, which will encourage our coaches and contribute to their development

2. Assist in the selection process of our coaches in upcoming years

SURVEY RESPONSES WILL BE KEPT CONFIDENTIAL
1.House League Team Number/Name, Division, Coach or Assistant Coach Name and Contact Information
2.The coach treated players fairly and provided equal opportunity to all players in all situations
3.The coach kept the team controlled and respectful on and off the ice
4.The coach was an appropriate role model, kept composure and kept winning in perspective
5. The coach taught skills and tactics appropriate to the age level and organized safe, fun and meaningful practices
6.The coach kept year positive, enjoyable, and rewarding for your player
7.The coach interacted and communicated in an effective and timely manner with parents
