We would like to ask you to participate in the survey of the ARCHIPELAGO program.

The purpose of this survey is to have a feedback on your experience with the ARCHIPELAGO project and the training-related activities.

Your participation in this survey is entirely voluntary. The survey includes 7 questions and sub-questions. Filling in the survey will take no more than 10 - 15 minutes.

The information you provide is confidential, except that anonymised quotes may be used.

Your name or any other personal identifying information will not appear in any publications resulting from this survey; neither will there be anything to identify your place of work.

The information gained from this interview will only be used for the above objectives, will not be used for any other purpose and will not be recorded in excess of what is required for the programme.

Only relevant program staff will have access to the survey data itself. These program staff will be bound by the principles outlined above.

There are no known or anticipated risks to you as a participant in this survey.
