You must be of 18 years or older to participate in this survey.
  • I have been given clear information about the study, and understand what is requested of me.
  • I understand that my participation is voluntary. I may refuse to answer any question and I remain free to withdraw from the study at any time without explanation.
  • I understand that all information from the survey will remain confidential to the researcher and that all information will be securely stored with all potentially identifying information removed.
  • I understand that none of the information I provide will be described or portrayed in any way that will identify me in any report on the study.
  • I understand that the findings of this research project will be submitted for publication with assurances for my confidentiality to be strictly maintained.
  • I am aware that I may ask any further questions about the research at any time by e-mailing the principal researchers.
  • I understand that participation is voluntary, and that I can leave this survey at any time should I choose to no longer continue with my response.
This study has been reviewed and approved by the UMBC Institutional Review Board (IRB).  A representative of that Board, from the Office of Research Protections and Compliance, is available to discuss the review process or my rights as a research participant.  Contact information of the Office is (410) 455-2737 or compliance@umbc.edu.
