* 1. The researcher requests your consent for participation in Australia's Speep Apnoea surveyto identify the prevalence and treatment options for sleep apnoea disorder. 

This consent form asks you to allow the researcher to capture the survey answers online and to use your comments to enhance understanding of the topic.

By proceeding to the survey, you confirm that:

• You understand that any responses/information you give will remain anonymous.

• Your participation is voluntary.

• This questionnaire asks for your preferences to be contacted which will allow the researcher to reach out to you.  If you choose not to, your information is not recorded.

• You consent that the data will be pooled and published.

• You consent to the anonymised data for a period of 10 years after the study has finished.
