Fill out this form to request an appointment. Provide 3-5 potential days that you are available. We will text you with the confirmed appointment time. If the times you provided are unavailable we will text you and ask for other options. Do not request same day appointments. Partial payment is required to reserve an appointment. Appointments will only be held for 24 hours without partial payment. Partial payments are nonrefundable and are applied to total cost of service. If appointment is missed, then partial payment will not be refunded nor will it be applied to future appointments. Payments are quoted in cash discount, all other forms of payment will receive a $2 or more increase of quoted price (i.e $60 = $63). Do not submit a payment, until after your appointment is confirmed.
