Thank you for coming to the 2023 SANYS Conference Award Nomination Form.
You can nominate someone you know for one of the awards by answering the questions below.  The awards are as follows:  Bernard Carabello Self-Advocate of the Year, Self-Advocacy Group of the Year, Advisor of the Year, Direct Support Professional of the Year and the Agency of the Year. 
 Also, please note the following nominations will not be eligible to be a nominee for an award:
 
-  Nominations from people who nominate themselves;
-  Nominations from a family member who nominate another family
    member;
-  Nominations of SANYS Board members or staff;
-  Nominations of person, group or organization from outside of New York  
    State.
You can nominate in one category, or multiple categories, but please make sure you include a description of why you think someone deserves an award for everyone you nominate.
 
All nomination forms must be sent to Cynthia Gilchrist, cgilchrist@sanys.org or mailed to SANYS, 500 Balltown Road Bldg. 12, Schenectady, NY 12304.  The due date to have the forms submitted is Friday, October 6, 2023.
