IPPOSI Patient Education Programme 2021 - Online Application Form

* Section 6 (motivation letter) does not include questions. Instead it has instructions for what to include in your letter of motivation. This letter is a mandatory part of your application - any application received without an accompanying letter of motivation is incomplete and will not be considered. Please see section 6 for full instructions. 

This application is not complete until you click 'Done' in section 7. If you do not click 'Done' once finished, the application is not saved and IPPOSI cannot access it. Please take care to do this upon completion of the application form's questions and declarations.
