Hello from Be the Source (formerly Foster Source), and welcome to our annual pulse survey! This survey is very important in ensuring that we are providing you, the caregiver, with the absolute best programs and services to assist you on your caregiving journey. We couldn’t do this without your participation! In addition to helping us, you will have the opportunity to enter a drawing for a $100 gift card (we're giving out 3)!
This survey will ask you which Be the Source programs you have used in the past 12 months and the impact they have had on your fostering/caregiving experience. We anticipate the survey to take about 10-15 minutes of your time.
This survey is optional. If you do respond, your answers are anonymous. If you opt to disclose personal details or any identifying information, these responses will be kept confidential and will not be shared outside of Be the Source unless permission is explicitly granted. Your responses below will in no way affect your access to any of the services or resources you receive or may receive in the future from Be the Source.
At the end of the survey you will be offered the opportunity to participate in a drawing for one of three $100 gift cards. Participation in this drawing is optional, and your entry will be recorded in a separate survey not connected to your responses.
