Food Allergies & Filling the Gaps

You have received this informal survey as a registered audience member of the 2013 Food Allergy Bloggers Conference. The purpose of this short questionnaire is to identify what this conference audience recognizes areas that need more attention with respect to food allergies.

Please respond to this survey by October 11, 2013.

Survey responses will be used for future conference planning and may also be shared with food allergy nonprofit organizations and the American Academy of Allergy, Asthma & Immunology and American College of Allergy, Asthma & Immunology.

• The survey includes 22 questions and takes about 10 – 20 minutes to complete.

• Completion of this survey is voluntary and respondents will remain anonymous.

• Responses will be reviewed by members of the FAB Conference Closing Group panel, the FAB Conference coordinators and also shared with the audience during the conference closing session with facilitated interactive discussion.

• The survey focuses on clinically diagnosed IgE mediated food allergies. Comments are also welcome from those dealing with food intolerances but please specify the difference in diagnosis in the boxes provided within the survey.
Please submit your completed survey by Friday, October 11, 2013.
