Thank you for participating in this survey about Kentucky's alternative education programs*. We are looking for feedback from the following stakeholder groups: alternative education program personnel, traditional K-12 personnel, students, families, and alumni of alternative eduation programs, state and public agency partners, and community and cross sector partners (including behavioral health professionals and career readiness/workforce development professionals).

This survey is being conducted by the Kentucky State Action Collaborative on High School Alternatives, a consortium of organizations working to improve educational opportunities for students ages 14-24 who are not well-served by traditional educational settings. Your responses will help guide policies and services designed to strengthen high school alternative education programs.

Your responses will be kept confidential and are voluntary. Survey responses will be stored in secure systems and accessed only by authorized members of the research team. Results will be reported only in summary form, and no individual participant will be identified. There are approximately 20 questions to answer.

To be eligible to be entered into a drawing for one of five $25 Visa gifts cards, you must complete the entirety of the survey and enter your contact information at the end. If you provide your name, your identifying information will not be part of any survey results and will be removed prior to analyzing results.

*What do we mean by alternative education? Alternative education programs in Kentucky serve students whose needs cannot be fully met in traditional classroom settings. These programs provide different pathways to graduation for students who may be at risk of dropping out, returning to school after leaving, or facing circumstances that require more flexible or specialized support.
