ODE Farm To School Procurement Grant - Baseline Report |
Introduction
Thank you for opting in to the ODE Farm to School Procurement Grant for Oregon Grown or Processed Food for school years 2017-18 & 2018-19. We are excited to work with you over the next two school years to purchase qualified Oregon grown or processed foods for your school meal programs.
In order to enroll you in the reimbursement procedure for the grant, you are required to submit information about your past purchasing history of Oregon products, promotion of Oregon products and where Oregon foods were served in your schools.
Data from this baseline report will help us make changes to the program to better serve your needs and help us measure the success of the ODE Farm to School Procurement Grant over the next two school years.
The survey should take you between 15 minutes to 1 hour to complete.
REMINDER: You will not be able to submit a reimbursement claim until we receive your complete baseline data report.
