Welcome to the Community Safety and Well-Being Survey

Some questions in this survey pertain to sensitive topics including mental health, suicide, substance use, and physical health services. This data is being collected to better inform the agencies that form the Community Safety and Well-Being Plan about the state of the community so changes to services can be made, if required. Graphs/charts will be made following review of the individual surveys in their entirety to reflect percentages and statistical data as a collective.
The provision of demographic information is optional, but the purpose of the collection is to determine if targeted outreach is required to ensure all voices in our community are heard.

Notice of Collection:

Personal information, as defined in the Municipal Freedom of Information and Protection of Privacy Act (MFIPPA), is being collected under the authority of the Community Safety and Policing Act, 2019, and in accordance with MFIPPA. Personal information collected in this survey will be used by Staff at the City of Stratford to share findings with the Community Safety and Well-being Partnership Council for the purpose of informing service levels that represent the state of the community.
