SDIRC in collaboration with the Learning Alliance has been utilizing FUNDATIONS as a supplement to our reading series "Wonders" for many years. As a district we want to make sure this program is meeting the needs of our students. We are asking you to complete this survey anonymously. Thanks in advance for your assistance with this. 
1.How many years have you implemented FUNDATIONS in the grade level you are currently teaching?
2.What specific components in FUNDATIONS make it unique from other reading programs?
3.The data from my students shows growth in phonics and phonemic awareness skills?
4.The training I received has prepared me to implement this program.
5.I use the data from the fundations assessments to guide my instruction.
6.Our grade level and district data meetings spend time discussing Fundations.
