INTRODUCTION:
Your TransFit Committee and your MDSC Executive Team, in conjunction with PayneWest Insurance, are seeking your input on both your perception of the culture of well-being at MDSC and what programs you would like to see.
Please take a few minutes to complete this brief survey and share your opinions. An excellent well-being program is one that you find meaningful. Help us to design a program for YOU. The survey should take no longer than 5 to 10 minutes and is COMPLETELY CONFIDENTIAL. The survey results are being collected and compiled by our partner - PayneWest Insurance; therefore MDSC will only receive two things: aggregate results and a separate list of the names of participants to be entered into a drawing for some fabulous prizes!!! Prizes will include items to help you relax and have fun, an important part of your overall well-being!
