YMCA School Age, Needs Survey

Thank you for your interest in a YMCA Before and After School Program! Please complete the questions below to indicate your families need. If you are interested in an existing school age program, your information will be forwarded to the centre supervisor responsible for the program. If you are interested in a new program at your child's school, we will add your name and contact information to a waiting list for a program.
