Dear LEAP Parents,
The LEAP Board (comprised of LEAP parents and the program director) would like to gather feedback about your LEAP experience.
This is the third year that the survey is administered and we hope to use your feedback to evaluate the program's performance and guide LEAP's improvement and strategic planning. Please respond to the items on the attached survey to share your thoughts and level of satisfaction with various aspects of LEAP including: programming, safety issues, staff, communication and leadership. We welcome any suggestions for continuous improvement! The results of this survey will be shared during LEAP parent meeting scheduled for May 17, 2017.
In order to make sure that the opinions of all families are heard, we are hoping for 100% participation from all LEAP families.
We ask that each household complete only one survey.
The 50-question survey is anonymous and you do not need to record your name, unless you want to. If you choose to share your feedback over the phone, your name will *not* be recorded on the survey. However, to help us interpret the feedback in a more focused manner, we would appreciate it if you provide the grade level of your child(ren).
